Please print and distribute the following forms to your team:
These forms must be taken home and signed. Please remind parents to review the documents carefully with their players to ensure everyone understands the expectations for both players and parents.
Signed Code of Conduct and Social Media Release Forms must be returned to the Registrar:

Medical Forms
Please print and distribute the following form to your team:
- Player Medical Information

Parents must complete the Medical Information Form and return it to you.

These forms must be kept on file and brought to every game/practice in case of a medical emergency.

You are responsible for these forms until the end of the season. If you are going to miss a game or practice please ensure someone is acting in your place and they have the forms present with them.

At season’s end, please return all medical forms to your coach for proper disposal.
